Health Records
You are required to have updated health records on file in Health Service. These must be documented on our forms in order to meet Illinois State Law. All health forms are due by December 15th.
For new incoming Chicago Campus students, these forms include:
Please completely fill out the forms. Please note that you need to allow time to schedule and complete a physical exam and that some of the forms need to be signed by your licensed physician.
