Health Records
You are required to have updated health records on file in Health Service. These must be documented on our forms in order to meet Illinois State Law. All health forms are due by July 15.
For new incoming Chicago Campus students, these forms include:
Please completely fill out the forms. Please note that you need to allow time to schedule and complete a physical exam and that some of the forms need to be signed by your licensed physician.

Health Service also has an online student health portal! Currently, students can schedule appointments with our campus doctor and message Health Service securely and directly through this portal. Additionally, students will soon be able to set up all other appointments and view their health records. We encourage all students to register for the student portal by creating an account once you have your Moody email account. Click here to register.
